Insurance & Self-Pay Fees

Insurance Clients

 I am ONLY in network with the following insurance plan:

  • Caresource Marketplace - Indiana

If I'm out of network with your insurance, you will be paying out of pocket for therapy services with me. I provide superbills upon request, and have partnered with a service called Reimbursify to make it easier to submit out of network claims to your insurance for potential reimbursement (please see last section below for more information). I also offer a limited number of reduced rate spots for self-pay clients who demonstrate financial need. 

If I'm in network, I use a third party billing assistant to submit claims to your insurance company for you. However, it is your responsibility to know your insurance coverage for mental health services, as you are responsible for any fees not covered by insurance.  Please be aware of any co-pay, co-insurance, and deductible as well as in-network and out-of-network rates. 

Self-Pay Clients — Services and Fees

Individual Therapy:

$160 - $180 / 50-minute session

Individual services are delivered 100% virtually and consist of our intake session (first session together) and any subsequent follow-up sessions. All sessions are 50 minutes in length unless we’ve discussed and agreed upon an alternative together.

Couple’s Therapy

$180 / 50-minute session or $325 / 90-minute session

Couple’s services are delivered 100% virtually and consist of our intake session (first session together) and any subsequent follow-up sessions. Options for 1.5 hour sessions are available on an as-needed basis.

Extended EMDR or Brainspotting Sessions

$325 / 90-minute session

Intensive EMDR sessions are an optional service for those who desire a more intensive experience with EMDR. These services are delivered 100% virtually and are 1.5 hours in length. These sessions are available to those who wish to engage in more intensive, specific EMDR or Brainspotting treatment for accelerated healing. This option is available after we’ve met for several 50-minute sessions to complete your intake, review your goals for treatment, and establish coping tools for use during our session.

Payment/Cancellation Policy

I use the Stripe payment processor on Simple Practice to process payments for services.

If I am unable to collect payment due to your card being declined or blocked, this may result in the cancellation of your upcoming appointments until your balance is paid in full.

  • All cancellations require 72 hours/3 days notice. You can call/text 260-799-1447, or email meg@lykkecounseling.com To avoid a fee, you may reschedule within the same business week, as availability allows. 

  • All late cancellations and/or no-shows are subject to the full fee of the session. For self-pay clients, this is your established self-pay rate, and for insurance clients, this is the contract rate I have with Caresource for services.

  • If you are more than 15 minutes late, your appointment may be canceled and a fee will be assessed at that time. If you accrue more than 2 late cancels/no-shows in a row, you may be removed from my schedule or discharged.  

  • If you are involved in a court-related case, phone calls or court appearances may incur additional fees. Letter writing or document preparation may also incur a fee.

Other Important Things to Note:

  • Lykke Counseling & Wellness, LLC does not have the capacity to seek preauthorizations or to submit out of network claims for clients at this time.

  • Superbills are available for those who would like to submit for possible out of network reimbursement. Please visit https://go.reimbursify.com/LykkeCounseling to submit out of network claims for services through Lykke Counseling & Wellness.

  • Lykke Counseling & Wellness cannot guarantee OON reimbursement. It is your responsibility to determine whether you have OON benefits with your insurance provider.

  • Notice in accordance with the No Surprises Act: You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
